11 November 2024: Clinical care for women seeking pregnancy after miscarriage | AJGP

OverviewSpecialist profile
Miscarriage affects at least 15% of clinically recognised pregnancies, and the emotional and physical toll it takes on women and their partners is profound. A peer-reviewed article co-authored by Associate Professor Alex Polyakov, Medical Director at Genea Fertility Melbourne, addresses one of the most common questions women ask after a pregnancy loss: when is it safe to try again?

The evidence is reassuring. Current research indicates that delaying conception after a miscarriage does not offer any tangible benefit, and conceiving immediately is considered safe for couples who feel ready. A shortened interval of fewer than three to six months may in fact be associated with a reduced risk of subsequent miscarriage, preterm birth, and other complications. The decision should ultimately be made in consultation with a healthcare provider, taking into account age, medical history, and emotional readiness.

Psychological care is just as important as physical recovery. Research shows that around 29% of people who experience pregnancy loss carry a significant mental health burden one month after the event, with nearly 20% still experiencing symptoms nine months later. Routine screening for depression using tools such as the Edinburgh Postnatal Depression Scale is recommended, alongside access to counselling and support services.

From a medical standpoint, around 70% of miscarriages result from chromosomal abnormalities and occur beyond a woman's control. Lifestyle factors including smoking, alcohol, caffeine intake, and weight can influence risk and are worth addressing before trying to conceive again. For women who have experienced two or more losses, investigation for recurrent pregnancy loss is now recommended under updated European guidelines.
