How Common is Male Infertility?

Male infertility is more common than many people realise. In fact, it contributes to about half of all infertility cases. Approximately 7-10% of men worldwide are affected by infertility, making it a significant issue that warrants attention and understanding.

What is a Semen Analysis?

A semen analysis is one of the primary tests used to evaluate male fertility. It involves examining a sample of semen under a microscope to assess sperm count, motility (movement), morphology (shape), and other factors like semen volume and pH. This test helps determine whether there are any abnormalities in the sperm that could be affecting fertility.

Increasingly, additional sperm tests are being performed together with the semen analysis, for example sperm DNA fragmentation analyses and tests of anti-sperm antibodies in the semen. Your doctor may order these tests either straight off the bat with the first semen analysis, or following an initial result that requires further assessment. 

How Do I Get My Sperm Tested?

Getting your sperm tested is straightforward. Your doctor will usually refer you to a lab where you will provide a semen sample, which should be produced by masturbation (unless otherwise arranged with your provider) into a sterile container which will be provided. This can be done at the lab or, in some cases, at home and brought to the lab within a specified timeframe (usually an hour).

What is a Fertility Test? Why Do Men Need to Take It?

A fertility test in men typically includes a semen analysis and possibly a hormonal profile, which measures levels of testosterone and other hormones that play a role in sperm production. Men need to take this test to identify any issues that could be preventing conception. Early detection of problems allows for more timely and effective treatment options.

What is the Process of Infertility Diagnosis?

The diagnosis of male infertility typically starts with a thorough medical history and physical examination. Your doctor may ask about your medical history, sexual development, lifestyle factors, and any symptoms you might be experiencing. Following this, a semen analysis is usually performed. Depending on the results, additional tests may be recommended, such as hormonal tests, genetic screening, testicular ultrasound, or a testicular biopsy.

What Are the Important Tests for Infertility?

The semen analysis is the cornerstone of male fertility testing. However, if abnormalities are found, further tests may be needed. These can include:
  1. Hormonal testing: To check levels of testosterone and other hormones.
  2. Genetic testing: To identify potential genetic causes of infertility.
  3. Testicular biopsy: A small sample of tissue is taken from the testes to examine sperm production directly.

What Should I Do If an Abnormal Semen Analysis Result is Found?

If your semen analysis results are abnormal, it’s important not to panic. Your doctor will discuss the findings with you and may recommend additional testing to determine the cause. Additionally, a diagnosis of male infertility is usually not made on the basis of a single abnormal semen analysis, so a repeat test will usually be requested. Sometimes, lifestyle changes such as improving diet, reducing stress, or avoiding exposure to harmful substances can improve semen quality. In other cases, medical treatment or assisted reproductive technologies (ART) like IVF or ICSI may be recommended.

What Treatment Options Are Available?

Treatment for male infertility depends on the underlying cause. Options may include:
  • Lifestyle changes: Improving overall health through diet, exercise, and avoiding smoking or excessive alcohol.
  • Medications: To treat infections or hormonal imbalances.
  • Surgical interventions: To correct anatomical issues or retrieve sperm directly from the testes.
  • Assisted reproductive technologies (ART): Procedures like IVF or ICSI that can help achieve pregnancy even with low sperm count or motility issues.

What Might Male Infertility Indicate About Other Aspects of My Health?

Male infertility can sometimes be a sign of other health issues, such as hormonal imbalances, genetic conditions, or underlying medical problems like diabetes or cardiovascular disease. It’s important to view fertility as a window into your overall health, as it can identify men at increased risk for a variety of medical conditions, and it’s important to undergo testing as directed by your doctor and remain in ongoing follow-up in the future.
